Print the application and mail it, along with your payment, to the address that appears on the application. ALL FIELDS ARE REQUIRED

OPERATORS:
First Route - $150.00 per year
Additional Routes - $75.00 each, per year

SUPPLIERS:
Basic Membership - $300.00 per year
Bronze Membership - $750.00 per year (includes 1/4 page ad in Convention Yearbook and membership directory)
Silver Membership - $1500.00 per year (includes 1/2 page ad in Convention Yearbook and membership directory)
Gold Membership - $2500.00 per year (includes full page ad in Convention Yearbook and membership directory plus a function at the annual meeting named in your honor)
Platinum Membership - $3500.00 per year (includes full page ad in Convention Yearbook and membership directory plus a function at the annual meeting named in your honor. Additionally, you will receive an ad on the MAMA web site with a hyperlink to your own web page.)

PLEASE NOTE ---

1. All Sponsors (Above Basic) and their sponsorship level will be listed on the Web Site and in any MAMA newsletters.

2. Members can request to pay in two installments.

3. Membership Dues are not deductible as charitable contributions for federal tax purposes but may be deductible as a business expense. As required by the Internal Revenue Code, MAMA estimates that the nondeductible portion of MAMA dues - the portion that is allocable to lobbying - is 60%. Therefore 40% of your MAMA dues is deductible for tax purposes.
